Choose a stakeholder group within a healthcare organization (i.e. clinicians, patient experience teams, IT, regulatory).

Outline a 4-hour professional development seminar on the Triple Aim and its operationalization in the health sector.

Note: Be realistic in the content you will present and anticipate the audiences needs.

Topic: Social Determinants of Health

Write a 250- to 300-word response to the following:
    After reviewing the course materials this week, describe a situation in which you witnessed social determinants that kept a person or group of people from receiving access to care or quality of care. The situation you witnessed could be from personal experience, a news story, or a story relayed to you from a friend.
    Include in your description of the situation how safe – effective, efficient – equitable, or patient centered – timely care were not delivered.
    Suggest two solutions to eliminate the social determinants health.
    Include your own experience as well as two citations that align with or contradict your comments as sourced from peer-reviewed academic journals, industry publications, books, and/or other sources. Cite your sources using APA formatting.
    If you found contradicting information to what your experience tells you, explain why you agree or disagree with the research.
