For years Doctors have struggled with the issues of quality/safety education pertaining to interns/residents (GME).

One tool often used is the Morbidity & Mortality conference (M&M) where actual patient cases are presented by the residents caring for them. In many instances the patient may have suffered a poor outcome, or even death.
