Ethical and Policy Factors in Care Coordination

Every person has the right to access health services that meet their needs. This includes people affected by homelessness. Accessing healthcare is a major issue for homeless people who suffer some of the poorest health in our society. Nurses know both the physical and mental health needs of the homeless community, as well as the gaps in service provision and models of good practice. We are here to work with the homeless population to improve your health and give health professionals the prospect of improving healthcare systems for the benefit of all.

Nurses Code of Ethics Impact on the Coordination and Continuum of Care

Nurses code of ethics serves as a professional commitment to society. It gives nurses direction to advocacy for the public population and homeless people are the most vulnerable to exacerbate the illness and allow treatable conditions like diabetes and hypertension to become life threatening (Bernstein, Meurer, Plumb, &Jackson,2015) Nurses are ethically complexity of health care system. Homeless are often care for by clinicians on the streets or in shelters and require intensive case management and care coordination including close engagement with local housing authorities, medical care professionals and centers, inpatient facilities and other programs. ( Jain, Baackes, &O Connell2020,P3).

The social Determinants of Health and a Precise an Accurate Interpretation of the Factors Contributing to Health, Health Disparities, and Access to Servies (evidence based)

A persons health is determined by many factors called the social determinant of health. Homeless individuals belong to lower socioeconomic status which is a significant barrier that make them struggle with negative assumption about their characters and circumstances. The Stereotype threat makes them hesitant to disclose their homeless status to hospital staff due to and health disparity.

National, State, and Local Policy Provisions that Raise Ethical Questions or Dilemmas for Care Coordination.

The best means of improving how accessible primary health care is to the homeless remains controversial. In a fair society there must be a fair access to top quality health services experience difficulty in gaining access to health care. Health care is our nations greatest challenge. The organizing, financing, and private, national, as well as local.

It is likely that delivery organizations that provide care for homeless special needs plans would include traditional clinicians and centers that provide care for homeless individuals, such as federally qualified health center; local, state, and federal agencies; and community organizations with experience addressing the needs of homeless individuals. The patient protection and Affordable Care Act(PPACA) is a national health care plan for reforming the health care system. Administered by the federal government; it is presented as an alternative to the private health insurance.

Governmental Policies Related to Health and /or Safety Affect the Coordination of care

According to Healing Hands article, care coordination has emerged as a method for increasing access to comprehensive health care, particularly for vulnerable populations such as people without homes(2016,p.1). Unfortunately, there are many homeless people who are too ill to reside in shelters or on the street. Given the complex lives if homeless people, in and out of shelters, and the chaos and crowded conditions that exist in shelters. It seems that issues related to housing, transportation, mental illness, medication management, complex medical care and lack of insurance make the coordination and continuum of car challenging.

I understand the people who become homeless, Health Care may not be the first thing to take off on a daily is not just important, but it is needed. Health promotion can be successful with the appropriate intervention method and professional staff as well as the continuity of services and goods available in the community.
